The World Health Organization (WHO) has recently declared the ongoing mpox outbreak in Africa a global health emergency. The outbreak is particularly worrying due to the rapid spread of a deadlier strain of the virus, known as clade Ib, in the Democratic Republic of Congo and its appearance in at least four previously unaffected African countries.

Mpox, formerly called monkeypox, is a viral disease related to smallpox. It spreads through close contact and can cause flu-like symptoms followed by a painful rash. The current outbreak is different from previous ones because it's driven by clade I, which causes more severe illness than clade II. The subtype responsible for most of the current spread, clade Ib, is a relatively new mutation that has adapted to human transmission, making it more concerning for public health officials. The danger of mpox varies depending on the strain and the individual affected. Clade I has shown fatality rates of up to 10% in some outbreaks, though recent cases have had lower death rates. Certain groups, such as infants, people with weakened immune systems, and pregnant women, are at higher risk of severe infections.

While mpox has historically been found in Central and West Africa, the current outbreak is spreading to new countries within Africa and has recently been detected in Sweden, marking its first appearance outside the continent. This geographic expansion, combined with the potential for international spread, has prompted urgent action from global health organizations. The outbreak is particularly challenging to monitor and control in rural areas due to limited access to diagnostic tools and laboratories.

Beginning in 2026, Gavi will establish a global stockpile of mpox vaccines. However, challenges remain, especially in terms of vaccine accessibility in affected African countries and the need for improved diagnostic capabilities in rural areas. WHO has emphasized that vaccines are only part of the response; containing the spread will also require enhanced surveillance, diagnostics, and research to fill gaps in understanding the disease.
